Visiting the dentist for the first time?
Please remember to bring the following with you:
- Please bring your dental insurance card (if you have one). You may also email us your insurance card ahead of your appointment so we can verify your insurance prior to your appointment.
- A driver’s license or ID of some sort.
- You can print out the following forms to fill out prior to our dental visit or complete a form we will send you (via text or email).
- You can book an appointment online, via the ZocDoc button or call our office at 703.451.3303 to schedule your appointment.

Sometimes life happens, either you run late or had an emergency and that’s okay. We make our forms available to you online for you to complete on your own terms.
